The Federal Perkins Loan Master
Promissory Note (MPN) and the Personal Data Form are necessary
steps in accepting a Perkins loan. All students intending to accept
a Perkins loan for the first time must mail in this form. Simply
download the PDF forms (below) to a computer's hard drive, print
and fill out, and mail it to:
