2015 Community Health Needs Assessment Project
Conducting a Community Health Needs Assessment (CHNA) is a requirement of the 2010 Affordable Care Act and part of the IRS 990 requirement for a not-for-profit health system. A CHNA provides a global view of health within a community and can serve as a catalyst to align expertise and resources to address unmet health needs.

2012 Greater Fargo-Moorhead Community Health Needs Assessment Collaborative
The purpose of the Greater Fargo-Moorhead Community Health Needs Assessment Collaborative (GFMCHNAC) is to develop a view of the population’s health and the prevalence of disease and health issues within the specified service area of Cass County, North Dakota and Clay County, Minnesota. Several components comprise the overall assessment, such as county health profiles and indicator maps of five states, special population profiles, and surveys of community leaders and residents. Minnesota Department of Health Statewide Health Improvement Program (SHIP), PartnerSHIP 4 Health 2009-2011
This evaluation includes five health initiatives that were implemented in Becker, Clay, Otter Tail, and Wilkin counties in Minnesota from June 2009 through June 2011.The initiatives studied are community and school physical activity, school nutrition, worksite wellness, and health care guideline implementation. North Dakota Strategic Planning Database Website: Survey of Stakeholders (2011)
This report presents findings of a survey of key stakeholders across the state regarding the value of the pilot strategic planning database that was created with the hope of enhancing the viability of communities through cooperative ventures that nurture and promote resource sharing. Data were collected between March 16, 2011 and April 1, 2011. The North Dakota Strategic Planning Project was initiated by the North Dakota Department of Commerce in 2006. View the report here.
